Form

Paper Live Scan request forms are no longer used.  You need to print the form from the State of California Dept of Justice website, Request for Live Scan Service, Form # BCH 8016.

Here is the link to the LiveScan form, print two copies and have one returned to the Coach Coordinator with an original signature from the Live Scan operator:

Appointment

49erYSL pays for the Live Scan service if you use the Placer County Sherrif's office in Auburn.
